Jon Erik Hexum Death By Russian Roulette
In 1984 on the set of "Cover Up", Hexum began playing with a Magnum handgun for an upcoming scene, unaware that it was loaded with one blank. This ultimately caused Hexum to play Russian Roulette with himself as he pointed the gun to his right temple and pulled the trigger. The force of the blank caused the actor's skull to propel into his brain, killing him.
